You have questions. Then the appointment starts, the conversation moves quickly, and the question you meant to ask slips away. A little preparation gives you something concrete to bring into the room.
This original Bridgett Renee Health worksheet helps you organize your thoughts before a medical appointment or wellness coaching conversation. Copy it into your notes, print this page, or share the link with someone who could use it.
1. Put your three priorities on one page
Write one sentence for each concern. What has changed? What would you like explained? What daily activity has become difficult? Start with the concern that matters most to you.
The National Cancer Institute recommends bringing written questions and asking the most important ones first. Its communication guidance also encourages asking for plain-language explanations and taking notes. Read NCI: Talking with Your Health Care Team.
2. Bring your medication and supplement details
Make a list of prescriptions, nonprescription medicines, vitamins, herbs, powders and other supplements. Include the product name, amount you actually take and how often you take it. Photos of labels can help you communicate the details.
According to the National Center for Complementary and Integrative Health, supplements can change medication effects or interact in harmful ways. Discuss the full list with your physician or pharmacist before making changes. Read NCCIH: How Medications and Supplements Can Interact.
3. Use this appointment worksheet
- My main concern: ____________________
- When it started and what changed: ____________________
- My three questions: 1. __________ 2. __________ 3. __________
- Medicines and supplements to review: ____________________
- Instructions I want clarified: ____________________
- Changes or symptoms my clinician says to report: ____________________
- Who to contact with follow-up questions: ____________________
- My next appointment or agreed next step: ____________________
4. Leave with a next step you understand
Before you leave, explain your understanding of the plan in your own words and ask the clinician to clarify anything uncertain. Write down who to contact and how. If you want someone to accompany you or help take notes, ask about the appointment arrangements beforehand.
Where wellness coaching fits
A coach can help you organize your questions, clarify your goals and think through everyday routines. Coaching does not diagnose conditions, interpret symptoms as a diagnosis, prescribe treatment or replace your licensed healthcare team.
